What is Housing 1000?
Housing 1000 is our community campaign to house 1,000 chronically homeless men and women in Santa Clara County by 2013. We are the local chapter of 100,000 Homes, a national effort to house 100,000 over the next two years.
Our volunteers travel throughout the county to add local homeless individuals and families to our registry via a short, simple survey to assess vulnerability to early mortality. We then work to connect the chronically homeless individuals we’ve identified through our survey with permanent housing and services.
Care Coordination Project
In November of 2011 we launched the Housing 1000 Care Coordination Project, where clients are provided intensive case management services focused on quickly finding permanent housing and stability. The Care Coordination Project team works tirelessly to remove barriers to housing, to ensure a swift and smooth transition from unhoused to housed, and provide the in-home support necessary for formerly chronically homeless men and women to maintain their housing and rebuild their lives.
Housing ONE
In February 2012, we launched our Housing ONE site, because formerly homeless people need more than just housing. They need to be enthusiastically welcomed into their new communities. With your help, we can provide used furniture, furnishings, and other items that truly make a house a home.
