Housing 1000 is a community campaign to house 1,000 chronically homeless men and women by 2013. Housing 1000 partners include Destination: Home, The County of Santa Clara, the City of San Jose, Community Technology Alliance, and the Santa Clara County Collaborative on Affordable Housing and Homelessness with funding from The Sobrato Organization, Silicon Valley Community Foundation, Applied Materials Foundation, eBay Foundation, The Health Trust, City of San Jose, County of Santa Clara and Destination: Home. We are the local campaign of 100,000 Homes.
In November of 2011 we launched the Housing 1000 Care Coordination Project, where clients are provided intensive case management services matched with permanent housing. The Housing 1000 Care Coordination Project (CCP) is led by our partner organization EHC LifeBuilders, who oversees the both the CCP and our funded case management partner organizations, including Downtown Streets Team, InnVision, and New Directions.
We’re leveraging all the resources we possibly can–from public, private and nonprofit sources in our community–to end chronic homelessness.
We’ve started a registry of local homeless individuals and families. In the coming months, we will be connecting the chronically homeless individuals we’ve identified in this survey with permanent housing.
But formerly homeless people need more than just housing.
They need to be enthusiastically welcomed into their new communities. There will be a need for communities willing to throw house-warming parties and to participate in ongoing outreach, as well as a need for new furniture, household odds and ends, and other donations in kind.
